Program consisting of a system
of coordinated health care
interventions,
communications and
education. The program
is intended for people living
with chronic illness and
conditions in which patient
self-care efforts offer a viable
opportunity to improve the
quality of life and maintain health.
Disease Management includes such diseases as CHF, Diabetes, COPD, End Stage Renal Disease, Coronary Artery Disease and Cancer to name a few.
The Goals of Disease Management are:
- Help avoid costly and debilitating complications of improperly treated diseases
- Assure that patients obtain the proper medical care needed to reduce health risks
- Adhere to your physician’s plans of care
- Promote self management to improve long term outcomes and reduce avoidable hospitalization and emergency care visits
- Provide the patients with the education, communication and intervention necessary to learn proper self management techniques
