Hospice care is an approach to providing for individuals who have a limited life expectancy and choose palliative (comfort) measures rather than curative measures.
Hospice addresses the physical, psychosocial and spiritual needs of the patient and family/caregiver as a unit of care.
An interdisciplinary team approach is provided consisting of nurses, home health aides, therapists, dietitians, social worker, physician, chaplain, pharmacist and volunteers.
The emphasis of the hospice program is to keep the patient with the family/friends and maintain comfort - physical, psychological, and spiritual. This care can be provided in the home, assisted living facility, or long term nursing facility.
Care is available through a 24-hour on call service - a nurse is available to the patient and can secure necessary medication or equipment on a 24-hour a day basis.
Inpatient and respite services are available when care at home is not manageable.
